RETURN INSTRUCTIONS

How do I send back recalled product? It’s easy!

After identifying your recalled lot numbers:

  1. Put all your recalled infusion sets in a box and seal it for shipping.
  2. Place the enclosed prepaid return label on your box.
  3. Drop off your box at any UPS store/location or call UPS at 800.742.5877 for free pick-up.

IMPORTANT: Continue your normal order cycle to ensure your therapy is not interrupted. We will replace all your recalled infusion sets free of charge. Replacement sets may take longer to receive than your normal shipments. All infusion sets you receive moving forward will be new and enhanced.

ONLY RETURN RECALLED INFUSION SETS

  • Do not return any used, open, or unsealed infusion sets.
  • Unrecalled product will not be replaced or sent back to you.
